My lesson plans change depending on the specific needs and abilities of the student(s) that I'm working with. A common outline that I follow however is:
A short review of what we worked on in the last session
Go over the basics of what we'll be doing in the current session and check for understanding of the new topics (if any)
Work on the new problems with the student (how much help is given depends largely on the students needs and abilities)
A short review of the different topics covered in the session (if more than one)
A short preview of what we'll be covering next session
