Student Agreement – Vocal Improvement Lessons

(By booking a lesson, I confirm that I have read and agree to the following):

1. Age & Consent
- If I am under 18, my parent or legal guardian will be present for the first 5 minutes of the first lesson and has provided written consent in advance.

2. Vocal Health Disclosure
- I have disclosed any current or past vocal health issues (nodes, reflux, surgery, etc.).
- If I am under the care of an ENT doctor or speech-language pathologist, I have obtained and can show written permission to take lessons.
- I understand the coach is not a medical professional and I am responsible for my own vocal health.

3. Scheduling & Cancellation Policy
- I understand payment is non-refundable once made.
- I may reschedule ONE time if I give at least 48 hours notice.
- Cancellations or reschedule requests with less than 48 hours notice = full lesson fee forfeited.
- No-shows or cancellations with less than 24 hours notice = 100% of the fee kept, no reschedule or refund.

4. Lesson Conduct
- I will be on time and with water ready.
- I will not eat during the lesson (drinking water is fine).
- I will ensure no background noise (pets, family, TV, etc.).
- I will behave professionally. Any harassment, inappropriate language, or disrespect will result in immediate termination of the lesson, no refund, and a permanent ban from future lessons.

5. Payment & Packages
- I understand single lessons and packages are non-refundable except in extreme circumstances (at the coach’s sole discretion).
- Unused lessons in a package expire 12 months from purchase date.

I understand that booking a lesson means I fully accept all of the above terms.
