I first have a quick chat with you to figure out what are your needs and goals.
Trying to get a passing grade in that last required math course you'll ever take if very different than making sure you fully understand all of the details of Calculus 1 knowing you'll eventually need to take Calculus 2 and 3.
I then pretty much follow these steps for each new concepts:
1. Go through the definitions - The most important thing in math is to know what each word means. While doing that, I also advise you on which definitions are most important to remember. Depending on your interest, I can also go into detail as to why things are defined the way they are.
2. I prod you a bit about the definitions we've just seen. I ask you the simplest questions possible just to make sure you have a good understanding. Depending on your interest, I can also ask you pathological questions that highlight the subtleties of the definitions and foreshadows what's to come.
3. We go through the provided exercises or I make my own. I let you do them on your own, but I ask that you explain your reasoning at every step. I'll be listening to make sure you're on the right track and I'll be there to answer any questions you have along the way.
4. I notice which questions were more challenging and I have you redo them or create new similar questions. I re-explain the concept in a different way if needed.
5. Once I think you fully grasp the concept, I'll give you some practice problem that you can do later on your own and we'll move to the next concept.
